Patient Directory
We maintain a directory listing patient name, location and a simple description of condition (i.e. satisfactory, poor, critical). We release this information to anyone who requests information about a patient by name. If you do not wish to be listed in the Hospital Directory we will honor your request.
By making the request not to be listed in the directory you are consenting that you do not desire any information released, even to family members who may or may not know you are here. When someone - anyone - requests information about you, we will not be able to acknowledge you are here. You may have phone service. However, mail and flowers will not be accepted on your behalf because of your requested status.
