BRIEF JOB DESCRIPTION: Assess the physical therapy needs of patients and families in the home environment. Develops plan of care for the patient in conjunction with the physician and other health care providers. Administers prescribed care according to physician orders. Educates caregivers on plan of care and provides support to the patient and family. Works with support services in obtaining needed supplies, equipment, and services for patients.

 

QUALIFICATIONS: Job requires specialized knowledge as to administration of treatments and maintenance of equipment. Must exercise judgment in administering and analyzing treatments. Education requirements are a Bachelor of Science degree and a diploma from AMA approved and accredited school of Physical Therapy. Successful completion of state board examination is required.

WORKING HOURS: PRN; working various days/hours, including weekends. Own means of transportation is required.
